Welcome to the 2021 Player Information Form (PIF) for StickStar Lacrosse!

This session will allow you to sign consents, complete contact information, provide Health Insurance information, and complete the Player Information Form for 2021. 
BE ESPECIALLY CAREFUL WITH GRADUATION YEAR!  It is your PLAYER'S GRAD YEAR and not the current year (unless he is a senior).

We heartily recommend that this NOT be completed on your phone.  Find five minutes when you can access a larger device and have all that we ask for close to you.

Our goal is for you to only fill this out once per year!  All the information that we gather is vital.  For a Registration to be complete, you must go all the way through to the end.  We will give you the same advice that we give our players - FINISH!  Thank you for your help!  Also, please, check that the information provided is CORRECT as it is used to populate rosters requested by college coaches and order apparel.  Finally, the Player Information Form (PIF) is NOT the same as the Summer and Fall Survey that we send out prior to each season.   

StickStar Lacrosse’s Dallas Select, Texas United, Texas Box Lacrosse and Oklahoma United are all about having fun, making friends, learning, ...and winning.  We look to compete and foster those parts of a player's game that cannot be taught during one-on-one lessons.  Only through actual game experience can many of the necessary lacrosse skills be learned.  Games provide the context!

Please, fill out the registration form completely.  All information requested is required and will be handled in the appropriate manner.

Deposits will be made via PayPal.  Always include a note on PayPal as to what the payment is for (e.g. Anthony Crimmins, deposit, Crab Feast).  PayPal always sends receipts via email would should be saved to a folder in case they need to be referenced.

Copies of insurance cards should be scanned in (both sides in pdf format) & included in this registration.

We heartily recommend that copies of any forms sent us be kept by the parents.  Also, they are a handy spot to keep record of payments made.

Each player must have registration with US Lacrosse that lasts through August.  If necessary, renew registration below and then return to this Registration Session to signup.  If you have not already signed up with US Lacrosse, please follow these steps:

• Step 1:  Obtain current US Lacrosse Membership
  • IF YOU DO NOT HAVE A CURRENT US LACROSSE MEMBERSHIP:  CLICK HERE to go to US Lacrosse Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Remember to register EACH of your players with US Lacrosse. Each player must have their own unique US Lacrosse confirmation number.
  • TO LOOK UP/RENEW A US LACROSSE MEMBERSHIP:  CLICK HERE to go to US Lacrosse Membership Lookup.
• Step 2:  Return to this registration and proceed. Later in this registration session you will need to enter the US Lacrosse Confirmation Code that you received in Step 1.
